I have a strong interest in endoscopic ultrasound (EUS) and endoscopic retrograde cholangiopancreatography (ERCP). Professor Atsushi Irisawa has given lectures in China on multiple occasions, through which I learned about the leading position of Dokkyo Medical University’s Department of Gastroenterology in the diagnosis and treatment of biliary and pancreatic diseases. Professor Irisawa also maintains a long term collaborative relationship with our department. Therefore, I wish to visit your institution for further training, to systematically learn standard EUS procedures and standardized ERCP techniques, and to apply and promote these advanced experiences back in my clinical practice.

私は超音波内視鏡(EUS)と内視鏡的逆行性胆管膵管造影(ERCP)にとても関心があります。入澤篤志教授はこれまでに複数回にわたり中国で講演されています。私は講演を拝聴し、獨協医科大学消化器内科が胆膵疾患の内視鏡診療において国際的に高い水準にあることを知りました。また、入澤教授は私が所属する西安交通大学第二附属医院の消化科と長年にわたり協力関係を築いておられます。獨協でEUSの標準的手技やERCPの体系的な手順を学ぶことで、その知識を臨床に応用し、先進的な経験を中国で普及できればと考えました。

我对超声内镜（EUS）及内镜逆行胰胆管造影术（ERCP）有浓厚兴趣。入澤篤志教授多次来华讲学，使我对獨協医科大学消化器内科在胆胰疾病内镜诊治方面的领先水平有了深入了解。入澤教授与我们科室长期保持合作关系，因此我希望能赴贵科进修，系统学习EUS标准操作和ERCP规范化技术，回国后应用于临床并推广先进经验。

In early summer, I arrived at Dokkyo Medical University. Professor Atsushi Irisawa arranged a well structured training plan for me. The supervising team of the Department of Gastroenterology – Dr. Yamamiya, Dr. Inaba, Dr. Maki Takumi, Dr. Fukushi, Dr. Yamamoto, Dr. Oike, Dr. Maki Ryuichi, Dr. Kashima, and others – took turns guiding me. From the very first day, I sensed a rigorous and persistent atmosphere. Every step was held to the highest standard, and these seemingly meticulous procedures built a solid foundation for patient safety. Under the patient guidance of all the teachers, I gradually adapted and made steady progress. I came to understand that beyond technical excellence, what truly matters is cultivating a “patient centred” mindset – every manoeuvre is a commitment to life. The teachers not only possess superb skills but also admirable character: meticulous in their work, they ease tension with cheerful laughter in difficult cases, and in daily life they are witty, knowledgeable, and caring towards juniors. Every teacher in the endoscopy unit made me feel welcomed. What they taught me went beyond techniques – it was optimism and a sense of responsibility.

6月初夏、私は獨協医科大学に到着しました。入澤篤志教授が私のために綿密な研修計画を立ててくださり、消化器内科の指導チーム（山宮知先生、稲葉康記先生、牧匠先生、福士耕先生、山元勝悟先生、大池研先生、牧竜一先生、嘉島賢先生ほか）が交代で指導にあたってくださいました。初日から厳格で粘り強い雰囲気を感じました。すべての段階で極限まで精度が求められ、これらの一見細かい手順の積み重ねが患者安全の基盤を築いていました。先生方の根気強いご指導のもと、私は徐々に適応し、確実に前進できました。研修を通じ、「患者中心」の考え方を持つことの大切さを痛感しました。一つ一つの手技に命への責任が込められているということです。先生方は卓越した技術をお持ちでありながら、その人柄にも深く敬服しました。業務中は細部にまで厳しく向き合い、困難な場面では朗らかな笑い声で緊張を和らげ、日常ではユーモアにあふれ、後輩にも温かく気を配っていました。内視鏡室のどの先生方も私に親切に接してくださいました。教えてくださったのは技術だけでなく、問題や難関を悲観せず克服しようとする前向きな楽観性と責任感の大切さでした。

六月初夏，我来到獨協医科大学。入澤篤志教授为我安排了紧凑的研修计划，消化内科指导团队（山宮先生、稲葉先生、牧匠先生、福士先生、山元先生、大池先生、牧竜一先生、嘉島先生等）轮流带教，第一天起我便感受到严谨执着的氛围。每个环节都要求极致，这些步骤构筑了患者安全的基石。在老师们耐心指导下，我逐渐适应并进步，领悟到技术之外更需“以患者为中心”的思维。老师们技艺精湛，人格令人钦佩：工作中一丝不苟，困难时以笑声缓解紧张，生活中风趣博识、关怀后辈。胃镜室的每位老师都让我温暖，他们教会我的不仅是技术，更是乐观与担当。

The academic atmosphere at the Department of Gastroenterology, Dokkyo Medical University, is exceptionally rich. The faculty members are rigorous yet approachable, and trainees from any background receive equal and patient guidance. The department maintains a busy but not overwhelming work pace. The endoscopy unit is equipped with state of the art devices and offers abundant clinical cases, making it an ideal environment for learning and improving skills. Notably, the faculty places great emphasis on standardisation — every step, from pre procedural preparation to post operative follow up, follows well defined protocols. This focus on standardisation is particularly worthy of study and adoption.

獨協医科大学消化器内科は、非常に濃厚な学術的雰囲気を有しています。先生方は厳格でありながら親しみやすく、出身を問わず、どの研修者にも平等かつ根気強くご指導くださいます。診療のリズムは忙しいものの、過度な圧迫感はありません。内視鏡室には最先端の機器が整い、豊富な症例に触れられることから、技術向上に最適な環境です。特筆すべきは、先生方が「標準化」を非常に重視されている点です。術前準備から術後フォローアップに至るまで、各段階で明確なプロセスが確立されており、この姿勢は特に学び、取り入れる価値があります。
