日本消化器内視鏡学会が実施するJGES “STARS” Programの一環として、当講座ではベトナムからの留学生 ドゥアン・グエン先生を受け入れ、5月15日から約10日間にわたり研修の機会を提供しました。帰国したグエン先生からのお便りをご紹介します。

“Thank you very much for the JGES STARS program for sending me to Dokkyo Medical University Hospital (DMUH), my time in the Gastroenterology Department of DMUH was truly an unforgettable experience for me.
In the late afternoon of the weekend, I first arrived at DMUH and was greeted warmly by Professor Atsushi Irisawa, Dr. Kazunori Nagashima, and Dr. Fumi Sakuma. Ten days of studying with detailed instructions from the Professors and all the instructors have broadened my horizon.
Doctors from three teams (Pancreas, ESD, and IBD) were in charge of instructing me on every case we encountered each day. It was the first time I observed Endoscopic Injection Sclerosis (EIS) of esophageal varices and gastric varices in controlling portal hypertension _ a difficult technique that was only performed in a few centers. Team Pancreas also taught me how to diagnose Chronic Pancreatitis by EUS and some interventional endoscopic procedures like ERCP to manage chronic pancreatitis complications.
Professor Kenichi Goda and Dr. Keiichiro Abe of the ESD Team trained me on how to use IPCLs changes in different types classified by the Japan Esophageal Society to recognize the early cancer of both the esophagus and the pharynx. Besides, I also had a chance to observe the ESD procedures in all regions performed by ESD Team.
Dr. Keiichi Tominaga and Dr. Mimari Kanazawa in IBD Team have taught me how to follow an IBD case and recognize the dysplasia changes of these lesions. IBD Team also performed double-balloon endoscopy in diagnosing the ulcer and stricture lesions in the ileum from sequential oral and anal intubations. These experiences help me a lot in taking care of my IBD patients.
There are no words to express my gratefulness for the time I was training at DMUH in the JGES STARS program. I hope that the JGES STARS program will continue and reach its hand to the limited resources regions in improving the endoscopic knowledge and skills in these countries.”　Doan Nguyen

　このたびは、JGES STARS Programにおいて獨協医科大学病院での研修の機会をいただきありがとうございました。獨協医科大学病院消化器内科での研修は、私にとって忘れられない経験になりました。まず、週末の午後遅くに到着した私を入澤篤志教授と永島一憲先生、佐久間文先生があたたかく出迎えてくださいました。この日から10日間、入澤教授をはじめとする消化器内科の先生方から丁寧な指導を受け、私の視野が広がりました。
　研修では、胆道膵臓疾患・ESD（消化管早期癌に対する粘膜下層剥離術）・IBD（炎症性腸疾患）の担当医師が、その日に対応するすべての症例について毎日指導してくださいました。
　門脈圧亢進症の制御における食道静脈瘤と胃静脈瘤の内視鏡的静脈瘤硬化療法 (EIS) を観察したのは初めてでした。EISは少数の施設のみでしか行われていない難易度の高い技術です。また、EUS（超音波内視鏡）による慢性膵炎の診断方法や、慢性膵炎の合併症を管理するためのERCP（内視鏡的逆行性短膵管造影）など、いくつかの内視鏡処置も教えていただきました。
　郷田憲一教授と阿部圭一朗先生は、食道と咽頭の早期がんを見つける重要な所見である、日本食道学会が提唱するIPCL（上皮内乳頭血管ループ）の形態を用いた内視鏡分類について指導して下さいました。さらにESD施行の全ての手順を観察する機会も得ました。
　富永圭一准教授と金澤美真理先生は、IBD症例を追跡し、病変の異形成変化を認識する方法を教えてくださいました。IBDチームは、連続した経口挿管と肛門挿管による回腸の潰瘍および狭窄病変を診断する際にダブルバルーン内視鏡検査も行っていました。これらの経験は今後IBD患者をケアする上で大いに役立ちます。
　獨協医科大学病院消化器内科での研修に感謝の思いでいっぱいです。今後もJGES STARS Programが継続し、資源の限られた地域の若手医師に留学の機会を提供することによって、各国の内視鏡医学の知識と技術向上につながることを願っております。　ドゥアン・グエン
